At Summer Game Fest 2026, 2K announced a story expansion for Mafia: The Old Country, entitled Man of Honor. It will release on August 14 for PlayStation 5, Xbox Series X|S, and PC via Steam and can be wishlisted now.

The trailer can be seen here:

Additionally, 2K describes the expansion as such:

Sicily – Winter, 1905

In the months since his initiation, Enzo Favara has proven himself a reliable soldier of the Torrisi crime family. Now the Don entrusts him and Cesare with a delicate assignment: assist Ennio Salieri, a man of honor recently released from prison and intent on reclaiming what is his. At Salieri’s side, Enzo is drawn deeper into the volatile underworld of the Valle Dorata, where loyalty is tested, debts are settled, and every move carries weight.

Continue Enzo’s story across two new chapters, featuring fresh environments, weapons, vehicles, and Charms, and exploring a previously unseen period in his early days as a soldato. These chapters can be played as part of the main campaign or accessed directly once you’ve progressed far enough in the story. In Free Ride, team up with Salieri to earn big by taking on a new set of exclusive jobs and extreme challenges. While exploring the Valle Dorata, discover new collectibles and hidden locations. This exclusive new content expands the existing Free Ride mode and is only available as part of the Man of Honor expansion. Salieri serves as your point of contact for these jobs, which will test the skills of even the most elite mafiosi.

Lastly, Mafia: The Old Country launched on August 8, 2025 for PlayStation 5, Xbox Series X|S, and PC via Steam and NVIDIA GeForce Now. The game features Sicilian voice acting and an Italian language localization for both in-game UI and subtitles.
